The Indiana State Department of Health (ISDH)maintains an Immunization InformationSystem known as CHIRP (Children and Hoosier’s Immunization RegistryProgram). Thepurpose of CHIRP is to collect, forecast, manage, and share vaccinedata to registered medicalproviders in Indiana. A provider may include private medicalfacilities, local healthdepartments, hospitals, schools, and pharmacies. The program is rununder the Indiana Code16-38-5 which allows for providers and their designees to accessdata from CHIRP. Inaddition an individual also may access their personal or theirdependents vaccine recordfrom either the registered CHIRP provider or directly from theIndiana State Department ofHealth.As an update to the existing process, ISDH has created theMyVaxIndiana ImmunizationPortal which enables a person to access a vaccine record from anycomputer. To begin thisprocess a registered CHIRP provider will create a patient ID number(PIN) which will grant aperson specific patient access to the record they have requested. Aparent or guardian couldrequest a PIN to view their dependents immunization history, or anyindividual could do thesame for their own record. They then have a number of options as tohow their data can beviewed and saved. This includes an option to print and save theOfficial ImmunizationRecord, download the record into a file format that may becompatible with their PersonalHealth Record system (PHR), or have it faxed or mailed to them.
